If you book a consultation, records review or Q&A session on the website at https://www.mypatientvoice.com (the “Site”), My Patient Voice will collect your name, email, telephone number and any message you write in the “Add Your Message” box. If you purchase a consulting service package, My Patient Voice may collect medical and health information from you, including but not limited to medical bills, explanations of benefits (EOBs), healthcare provider release forms, appointment of representative forms and other related documents.
