Protecting your private information is our priority. This Privacy Policy governs data collection and usage of the website, software, platform, products, and services (collectively, "Services") provided by My Patient Voice LLC, a California limited liability company, and its affiliates (collectively, “My Patient Voice”). My Patient Voice is also referred to herein as “we,” “us,” or “our.” By using the Services, you consent to the data practices described in this Privacy Policy, our Terms of Use, and, if applicable, our Terms and Conditions for Online Sales. If you do not agree to any part of this Privacy Policy, then you should stop accessing the Services.

1. Collection of Your Personal Information.  

My Patient Voice may collect personally identifiable information, such as, but not limited to, your email address, name, physical address, telephone number and IP address. If you purchase our Services, we collect and store your billing and credit card information. The information that we collect and store often depends on the type of Service that you use. For example, if you register with My Patient Voice, we may collect and maintain your account information such as login name, password, email address and contact information. If you book a consultation, records review or Q&A session on the website at https://www.mypatientvoice.com (the “Site”), My Patient Voice will collect your name, email, telephone number and any message you write in the “Add Your Message” box.  If you purchase a consulting service package, My Patient Voice may collect medical and health information from you, including but not limited to medical bills, explanations of benefits (EOBs), healthcare provider release forms, appointment of representative forms and other related documents.  
 

My Patient Voice may also collect anonymous demographic information, which is not unique to you, such as your Postal Code, age, gender, preferences, interests, and favorites.  Information about your computer hardware and software may be automatically collected by My Patient Voice. This information can include your IP address, browser type, domain names, access times, operating system, cookie information, referring website addresses, and Internet Service Provider. My Patient Voice may also collect geolocation information based on visitors' IP addresses. My Patient Voice may also collect information about your use of our Site, such as pages visited, links clicked, text entered, and mouse movements.
 

Please keep in mind that if you directly disclose personally identifiable information or personally sensitive data through the Services to third parties, such as third-party payment providers (i.e., PayPal, Apple Pay, etc.), this information may be collected and used by them. My Patient Voice encourages you to review the privacy statements of entities or services you access from our Service so that you can understand how those entities collect, use, and share your information. My Patient Voice is not responsible for the privacy statements, privacy practices, or content on websites or services outside of the Site.

My Patient Voice collects and uses your personal information to operate and improve our Services, including the delivery and performance of the Services you have requested or purchased.

My Patient Voice may, from time to time, contact you on behalf of external business partners about a particular offering that may be of interest to you. My Patient Voice is not a “covered entity” as defined under the Health Insurance Portability & Accountability Act (HIPAA).  My Patient Voice does not comply with the provisions of HIPAA or its similar state counterparts.  Any medical or health information collected by My Patient Voice is maintained, used, accessed, and disclosed in accordance with this Privacy Policy, the Terms of Use, the Terms and Conditions for Online Sales, and any other services agreement between you and My Patient Voice.
 

6. Children Under 13

Our Services are not intended for use by children under the age of 13, and such use is prohibited by our Terms of Service. We do not knowingly collect personally identifiable information from children under 13. If you become aware that a child has provided us with personal information, please contact us as set forth in this Privacy Policy.
 

7. Changes to This Privacy Policy

My Patient Voice may change the provisions of this Privacy Policy at any time and will post the most up-to-date version on our website. We may provide notice by email to you at the last email address you provided us, by posting notice of such changes on our sites and applications, or by other means consistent with applicable law.
