Service Description
This Agreement includes every service I offer except the 1 hour conference call. Rather than "a la carte" pricing, you pay a $250 deposit, which is deducted from any savings or reimbursement I procure for you. I will bill for 20% of the savings rewarded to you. If no savings are achieved, you will not pay the 20% fee. When you indicate you wish to engage my services, a contract and invoice for $250 will be forwarded to you via email. Work will commence after your deposit and signed contract is received.
Cancellation Policy
For Consultancy Agreement: To cancel your contract prior to the conclusion of your settlement, the $250 deposit will be forfeited. Pending rewards or reimbursement you receive as a result of my work will be billed at the 20% rate. Communication with your providers and insurance company will cease, and any project notes provided to you. It will be your responsibility to remove Jonna Jerome & My Patient Voice from any Release of Personal Information on file. All other services: Please cancel or reschedule your appointment 24 hours in advance.
